회전근개(Rotator cuff)의 MRI 소견

1. Rotator cuff의 normal finding

2. Rotator cuff pathology

1) Tendinopathy(Tendinosis)

① More common with age

② Increased signal within a normally sized or thickened tendon on T1W1 

③ Not as intense as fluid on T2WI 

④ Invariably seen at the site of ruptured tendon 

⑤ No active cellular infiltration -> No acute inflammation

2) Tear : Partial thickness, Full thickness, Complete

① Partial thickness tear(minute tear to high-grade partial tear) 

Classified based on the depth and the site of the lesion at arthroscopy 

Grade 1 : less than 3mm // Grade 2 : 3 to 6mm deep // Grade 3 : more than 6mm deep

* Rim rent tear : Articular side tear의 일종 

RCT 중 가장 흔한 형태로 RC가 GT에 부착하는 부위의 파열입니다(ABER view의 Coronal view에서 정확히 진단).

② Full thickness tear(Tendon discontinuity) 

Intense SI on T2WI d/t fluid-filled gap : Fluid in subacromial bursa,  Retraction of torn margin, Muscle atrophy 

Moderate to low SI d/t severe generation

3) Musculotendinous Retraction

4) Fatty atrophy

* Infraspinatus tendon tear

* Subscapularis tendon tear(Trauma, Anterior dislocation, RC massive tear & Biceps subluxation, Subcoracoid impingement)

Axial or Sagittal에서 T2 or T1 FS MR arthrogram 선호됩니다.

* Subcoracoid bursitis

* Subcoracoid triangle sign

* Intramuscular cyst
