무릎의 이학적 검사(Physical examination)

1. Inspection

1) Anatomic deformity(Alignment)

① 어느 한 쪽의 간극을 없앤 상태에서 한 쪽의 간극이 2cm 이상 있으면 각각 내반슬, 외반슬이라고 진단

② 내회전각이 클수록 대퇴골 경부의 염전은 크다고 판단 

③ 고관절 외회전각과 내회전각의 합계가 90º 이상인 경우는 고관절의 instability가 있다고 진단

2) Gait abnormality 

3) Skin & Subcutaneous : Swellings(B&M tumors, Ganglions & Cysts), Sinuses, Psoriasis of skin, Extraskeletal involvement in RA, Infections(abscess)

4) Muscle : Swelling(B&M tumors), Extraskeletal involvement in RA, Muscle abscess, Calcification in the muscle, Weakness and Paralysis

2. ROM

1) Note angle of knee while supine 

2) Passive ROM도 check 

3) Flexion : 135º, Extension : 180º 

4) Hyperextension >= 10º (not uncommon) 

5) With knee flexed 30º  

① Internal rotation 30º 

② External rotation 45º 

3. Patellofemoral assessment

1) Important to assess how the patella moves when the knee is flexed & extended 

2) Palpate superior, inferior, medial & lateral patella facets 

3) Compression test 

4) Integrity of patella & quad tendon(Patella tendon tenderness) 

① Flexion tenderness(+) : Patella tendon lesion 

② Extension tenderness(+) : Patella tendon or IPF lesion

5) Patellar apprehension test : Tests for patella subluxation or dislocation 

① (+) test is verbal or facial apprehension from the athlete, or an attempt to contract the quadriceps to avoid dislocation.

② Pain : Medial & Lateral retinaculum에서 유래하는 것

4. Effusion(Compare this with ‘good’ knee // Rapid(<1 hour), Delayed(24-36 hours))

1) Intra-articular effusion

① Rapid onset of edema : less than 24 hours // Ligamentous, Fracture, OCD 

② A slow onset is usually indicative of a meniscal lesion.

2) Extra-articular effusion

① Bursal edema : Focal effusion, Warm to touch, Localized inflammation 

② Patellar - femoral 

③ Soft tissue contusion 

④ Medial or lateral collateral ligament trauma

⑤ Ballotable patella : 다량의 삼출액 때문에 슬개골을 눌러도 삼출액에 의해 슬개골이 떠오릅니다.

⑥ Milking an effusion : 소량의 삼출액을 내측으로 모아서 삼출액이 고인 부위를 누르면 삼출액은 외측으로 이동하여 볼록하게 됩니다.

 

5. Special test

1) Knee instability test : Lachman, Pivot-shift test, Varus, Valgus stress test 

2. McMurray test 

3) Joint Laxity test : Finger, Thumb, Elbow, Knee, Ankle
